By selecting you acknowledge and agree to the following terms and conditions regarding the childcare services provided by The Event Nanny, LLC

Consent for Childcare: I authorize my Child/Children to participate in the childcare services offered during the event.

Health and Safety: I understand that all reasonable precautions will be taken to ensure the health, safety, and well-being of my Child/Children during their participation in the event. However, I acknowledge that the event organizers and staff are not responsible for any injury or illness that may occur.

Emergency Treatment:  I understand that every effort will be made to contact me before any medical treatment is administered.

Release of Liability: I release and hold harmless the event organizers, staff and any associated personnel from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, that may be sustained by my Child/Children during their participation in the event, except to the extent caused by the gross negligence or willful misconduct of the organizers or providers.

Emergency Contacts: I have provided accurate contact information for use in the event of an emergency involving my Child/Children.

Photo/Video Release: I grant permission for photographs and/or videos of my Child/Children to be taken during the event for promotional or informational purposes related to future events.

I have read this waiver form in its entirety and understand its contents. By signing below, I acknowledge that I voluntarily agree to all the terms and conditions stated herein.
